Good news: We've streamlined our process of requesting a new-patient appointment to make it easier.

 

First StepComplete our New Patient Appointment Request Form

This form will help to confirm that we can see you in our practice, and provide us with some basic information to get started.  

Note that due to contract and legal restrictions, we are not able to accept all insurances in our practice, and in some cases, you may not be able to be seen at all.  We will do our best to identify this up front for you, so you can seek services sooner.  

After submitting the New Patient Appointment Request Form, you will receive an email within 1 business day with information about accepting your insurance, and your next steps.

Second StepComplete our Online Patient Registration and History Form

*It is recommended that you not complete this form until you have completed Step 1 and received instructions to continue to this step.  

This detailed form provides all the information needed to schedule you for your first appointment and to be seen on the date/time of your first appointment (to be scheduled in step 3).  By completing the Online Patient Registration and History Form at this step, you are able to work at your pace, and save valuable time at your appointment.  You will not need to complete additional forms at your first appointment!

Important Reminders:

  • Save your Registration Number that appears at the top of each screen once you've started
  • Save your Password!
    • You may be asked to provide your Registration # and Password to ensure that we receive your submitted form.  This information will only be requested if we have a problem in receiving your form.
  • Take your time.
    • You can always save and go back, so be sure to complete all sections and questions that are relevant to you.
  • Submit!
    • Be sure to Submit (on the final Review tab)!  You WILL receive a confirmation once your  form has been submitted.  
    • This is important to ensure that we receive your information in order to schedule your appointment.

Once we receive your New Patient Appointment Request Form and completed Online Patient Registration and History Form, we will create your chart and call you for Step 3.

Third (and final) Step:  Schedule!

Once we receive your completed New Patient Appointment Request Form and Online Patient Registration and History Form, we will create your chart and call you to schedule your appointment and collect your deposit.  

This phone call should only last a few minutes, since you have already provided us all the information we need.  

  • Be Flexible
    • New Patient Appointment times are often the most restrictive to schedule.  Once you have been seen, additional times may be available for follow-up appointments, depending on your provider.
    • Our Staff will do their best to find a date and time that works best for you.
